Regardless of how good an organisation is positioned, accidents and incidents can not be completely eliminated in advance. By the time such an event eventuated, it should be investigated best possible, to identify all causes by using the root cause analysis, which became effective and allowed the event to happen.
Soonest possible after the event occured suitable corrections
should be specified and circulated to all members of the own organisation to ensure that the same event can not happen again.
After successfully completed investigation all identified causes must be evaluated to define preventive actions. These must be implemented into the organisational structure and evaluated for effetiveness after some period of time.
